Medical Social Work Consultant 1

 

Under the direction of the Executive Director of the Mission Critical Unit within the Office of Long-Term Care Resiliency, this position will serve as the subject matter expert for the Mission Critical Team as it pertains to resident rights, abuse prohibition, levels of care and appropriateness, care planning, discharge planning, resident financial resource allocation, psychosocial counseling, and addiction services, including maintaining a knowledge of applicable state statutes, regulations, policies, and procedures. Duties will include: touring the identified LTC facilities to observe staff knowledge/abilities; overseeing the counseling and guidance to patients and families; providing assistance with problem resolution to staff about residents and families; advising staff on the complaint/ grievance process; evaluating the adequacy of outreach services provided to residents; sharing best practices/operational tools with onsite social work staff; assessing that residents are in the appropriate setting for the level of care needed; assuring that staff are safely discharging residents to the appropriate environment; recommending improvement and providing educational tools/resources to onsite staff, reviewing the facilities’ quality assurance practices/policies; making recommendations based on resident council minutes; evaluating remediation process; and completing reports.

This position is primarily a field position, and will be required to travel to long term care facilities throughout the state. If operational need permits, employees will be assigned to regional locations.

 

This position may be eligible to participate in the Department's pilot telework program, which offers eligible employees the opportunity to work remotely for up to two (2) days per week, as approved by management.
